AM I A CANDIDATE FOR CHRONIC CARE MANAGEMENT?

Your primary care provider can help you determine if chronic care management is a good choice for you. You can enroll in the program at one of your regularly scheduled visits, including our Annual Medicare Wellness Visit. A comprehensive care plan that addresses your needs will be developed and maintained during your participation in the Chronic Care Management Program. Regular check-ins with your care coordinator to review progress, update your care plan, coordinate care in the health care system and with community resources, and answer questions about your health care.

Patients can enroll in the Chronic Care Management Program during a visit with their primary care providers. After enrollment, care coordinators work with patients by telephone. There is a charge for chronic care management services. Participants with secondary insurance or Medicaid often have no out-of-pocket expenses.

• Alzheimer’s disease and related dementia
• Arthritis
• Asthma
• Atrial fibrillation
• Cancer
• Cardiovascular disease
• Chronic obstructive pulmonary disease (COPD)
• Depression
• Diabetes
• Hypertension (high blood pressure)
